The patient or receptionist completes a pre-printed personal charge sheet from PMDS with all the relevant details of the patient, i.e. name, address, medical aid details etc.
The doctor consults with the patient and only the required documents of the patients diagnosis / procedure / dispensed medicine details will appear on the same charge sheet.

• Software System fees.
